Please complete this form, listing each student on your team. Both the coach and the school's principal should sign the form. This document should be turned in during coach sign-in on event day.
Code of Ethics: 2024-Code-Of-Ethics
The Code of Ethics form has a (group) signature page that MUST be signed by each student, and all adults who will be attending the tournament, including coaches and parents. This document should be turned in during coach sign-in on event day.

Each student/parent should sign a personal copy of the UA Publicity Release. These documents should be turned in during coach sign-in on event day.
Coaches - Regarding Publicity Release:
Your welcome packet will contain brightly colored stickers - please give students who have completed the Publicity Release Forms a sticker and ask them to apply it to a prominent and visible place on their shirt or jacket. We will only take photos of students with stickers clearly showing. Students without a release must not receive a sticker, please and thank you. UA will assume stickered students are fully released (approved/allowed) for photography and publicity.
